Health insurance is now available for all. Get covered! You and your family can apply onsite for affordable health insurance! What to Bring?
1. W-2 or last years tax return, birth certificate or legal resident card
2. Proof of US citizenship/residency status, social security number
3. Proof of property/assets such as current bank statement, vehicle registration
4. Information needed to enroll family members, number of people enrolling, birth dates, social security numbers, and home zip code.
