Encouraging families, individuals, business owners and sole proprietors to GET ENROLLED for affordable health insurance.  Walk up enrollment days now through January 31, 2015.

Fridays – 11 AM to 7 PM
Saturdays – 11 AM to 7 PM
Sundays – 12 PM to 6 PM

LOCATION: Upper Level Near Macy’s Entrance, Arden Fair Mall, 4322 4th Avenue, Sacramento.
Educators and enrollers on-site – bring these items with you for new enrollment or renewal: California I.D. or driver’s license; utility bill; passport or legal residence card/certificate; social security card, 2014 taxes or paycheck stub.
