InnovAge’s PACE center is a partnership with Adventist Health and Eskaton

DENVER, Colo.— A new option is now available for Sacramento-area seniors. In partnership with Adventist Health and Eskaton, InnovAge’s new Program of All-inclusive Care for the Elderly (PACE) is designed to keep seniors living in their own homes and communities for as long as safely possible. The program is an alternative to nursing facilities. Participants are primarily dually eligible for both Medicare and Medicaid, though U.S. military veterans and others can also qualify.

The new PACE center is located at 3870 Rosen Court in Sacramento. While the building itself is closed due to COVID-19, the program is open and enrolling seniors. Care is being provided via telehealth an in-person visits, along with wellness calls, home-delivered meals, medications, and more.
