Social Security Number for applications who are US citizens, or document information for immigrants with satisfactory status who need health coverage.
Proof of California residence.
Federal tax information
Information about health coverage that you or anyone in your household gets through an employer.
Plus, we will help you find out if you qualify for financial assistance.
