Opportunities to recognize people for great work are few. Do something good for yourself and someone else.  Take a moment to nominate someone TODAY!  The Healthy Eating, Active Living (HEAL) Collaborative would like to recognize those working to educate, engage and empower low-income populations to eat healthy and be active.  A Champion for Change can be anyone that has assisted a low-income population in making healthy changes, big or small, with a lasting impact.

Below are a few examples:

· Leading a successful nutrition education or physical activity program
· Encouraging healthy food options and healthy fundraisers at schools.
· Advocating for policy or environment changes that support healthy eating and active living.
· Helping to create community gardens or farmers markets.
· Educating community members to create changes in their communities that increase access to fruits and vegetables and/or physical activity.

Please take a moment to recognize a Champion for Change that is making a difference to improve the health of low-income populations, and tell us their story.  Use the attached form (pg. 2) to write one page (maximum) about how they are supporting healthy behavior change and submit to the fax number below, in person, or by using the online form here  by 4:00 pm on March 9th, 2011. Please be sure to include their contact information and yours.   A Nominations Committee will select a total of 5 “Champions for Change” who will be recognized and have the opportunity to share their success at the Healthy Eating Active Living Collaborative Sharing Forum on March 31, 2011 at Sacramento City USD Serna Center.
